To file a grievance at our agency, please complete the following form and submit it to our office.
LA County Department of Mental Health
LA County Department of Mental Health has an Office of Patient Rights that advocates to protect the rights of mental health care recipients. Clients with concerns can contact the Patients’ Rights Office at (213) 738-4888 or (800) 700-9996. Information about their Beneficiary Client Grievance Forms and Grievance Appeal Procedures are available on their website which is available in the link below.
